Emma Sky, who advised the US military during the conflict in Iraq and helped rebuild the country, talks to Iain Dale about her experiences there.
Sky - who was awarded an OBE for her work in Iraq - told Iain: "Nothing that happened in Iraq from 2003 onwards was inevitable.
"After the invasion there was no plan for how to manage the occupation of the country."
She also told Iain that former Prime Minister Tony Blair had to bear his fair share of the blame for the conflict and what had happened in Iraq since.
